He said the accident happened at 12.30pm when Anwar was being transported back to the Sungai Buloh prison after being hospitalised for four days at Hospital Kuala Lumpur (HKL).
He also said that all the cars had managed to escort Anwar back to prison after the incident.
The lawyer added that Health Ministry deputy director-general Dr S Jeyaindran was scheduled to visit Anwar this evening to assess if further treatment was required.
Sivarasa said the vehicle carrying Anwar had to brake sharply to avoid an oncoming auto, causing another escort vehicle from behind to knock into it.
"Datuk Seri Anwar fortunately did not suffer any serious physical injuries but was shaken up and has taken pain killers after being examined by the doctor at Penjara Sg Buloh".
"(Subsequently, ) the auto carrying Anwar suffered damage mainly to the back of the vehicle", he said in a statement here today.
His daughter Nurul Izzah Anwar said her father was warded due to his erratic blood pressure, as well as to undergo a routine physiotherapy for an old shoulder injury.
In May 2014, Anwar and his wife, Datuk Seri Dr Wan Azizah Wan Ismail, survived a head-on collision with another vehicle in a minor road accident that left the former with a fractured arm.
