Competing in her first Grand Slam since taking a year off to have her first child in December, Azarenka is bidding to become the first mother to win Wimbledon in 37 years.
Venus opened up a 4-1 lead in the first set, only to see Osaka respond with three straight games to knot things at 4-4, before the set ultimately went into a tiebreaker. Friday's match was only her fifth.
Victoria Azarenka continued her comeback to the tour with a 3-6, 6-1, 6-4 triumph over hometown favorite Heather Watson.
Prior to this year's event, Azarenka had been 0-8 in Wimbledon matches were she had dropped the first set.
The 32-year-old American was being treated in a nearby private hospital where she was taken on Thursday after her knee appeared to buckle as she approached the net in her second round clash with Sorana Cirstea on Court 17.
The two-time Grand Slam champion will next face either Simona Halep or Peng Shuai on Monday in the fourth round.
Making her 20th appearance at Wimbledon - the most among active players - Venus drew from all her years of experience to edge past the determined 19-year-old, 7-6 (3), 6-4.
Osaka won the first three points of the breaker, but the 10 seeded Venus rattled off seven straight points capped off by a forehand victor to take the opener. Before that, sixth-seeded Johanna Konta was facing Maria Sakkari.
